Provide services to emotionally disturbed children and their families as part of a collaborative team within Olmsted County's Youth Behavioral Health. Ideal candidates will possess relevant experience working with children and families, knowledge of community resources, excellent communication and listening skills, and the ability to work with families of various backgrounds.
Essential Functions and Responsibilities:

  • Develop an assessment to identify the emotional, physical, social, and educational needs of the child and the supports necessary for the family within the context of family, school, and the larger community to enhance child well-being in compliance with Minnesota State Statute.
  • Develop an Individual Family Community Support Plan (IFCSP), in consultation, with each family that would address the barriers, risk factors, needs identified, and develop goals , including a crisis plan, family support, and referral assistance
  • Provide direct service consistent with the IFCSP and needs of the child. Coordinate services with family, community, school, therapeutic foster homes, and other resources in the community. Coordinate internal services as needed.
  • Complete required documentation for Targeted Case Management billing purposes as appropriate in the client's electronic health record in a timely manner.
  • Complete all related documentation as directed. Documents all services contacts into SSIS in a timely manner including face to face contacts, collateral and networking contacts, correspondence, and maintains the case records in accordance with the agency and regulatory standards and requirements.


  • Bachelor's degree in social work, licensed or licensed eligible required.
  • Desire previous human service experience working with children and families.

Knowledge, Skills and Abilities:

  • Knowledge of family centered case management and principles of social work practice are required.
  • Possess the ability to work effectively with diverse populations, to work within a team environment, and effectively communicate with colleagues, co-workers, clients, and referral sources.
  • Knowledge of children's mental health issues desired. Must possess computer literacy.

Special Requirements:

  • Must have a valid driver's license and dependable transportation to use for work purposes.
  • Willingness and ability to work additional time if needed to meet crises and deadlines.
